Martha joined AccessHealth Spartanburg in 2016 as a Community Health Worker for the Healthy Outcomes Program (HOPS). Martha is responsible for providing direct client care through case management, care navigation and coordination of services. Martha conducts medical, social, and medication assessments to create individual care plans for each client and assists with referrals by making connections to other community resources. In addition, Martha provides education to clients to prevent chronic disease and live a healthier lifestyle. She also conducts home visits and escorts clients to medical appointments. Martha sets no limitations to overcome barriers to achieve client’s needs.
Martha brings 13 years of experience working as an advocate and patient benefits counselor in a community health care clinic that served a large and diverse population. Martha is bilingual Spanish speaking and is part of the Hispanic Alliance Committee.
Martha is originally from Mexico and has lived in South Carolina for the past 23 years. She has two daughters and one son. In her free time, she enjoys spoiling her three adorable granddaughters with goodies!
